For insurance claims or civil proceedings, you can apply for a copy of any existing collision reports the police have made or hold, or request an interview with the police officer who dealt with the road traffic collision, as long as it has been investigated.

To do this you'll need a Traffic Case Reference number.

They look like this - XX/0000000/year or xx/000000000/year.

Please note: A CAD number is not the same as a Traffic Case Reference number.

If you have a Traffic Case Reference number then download and complete the 518 form.

You’ll need:

  • the traffic case reference number
  • insurer or solicitor’s name, phone number and email address
  • insurer or solicitor’s full address including the postcode (this is where the copy of the report will be sent)
  • the date and time of the incident
  • the location and postcode of the incident

How long does it take?

If you’re requesting a copy of the report it can take up to 12 weeks from the date that we bank your payment to process your request.

If you’re requesting the details of a third party it can take up to 21 days from the date that we bank your payment to process your request.

Please note: Collision accident reports can only be released once the case has been closed.

If you don’t have a Traffic Case Reference number

If you don’t have a Traffic Case Reference number then you’ll need to complete a 518a form to request it.

You’ll need:

  • your CAD number
  • insurer or solicitor’s name, phone number and email address
  • insurer or solicitor’s full address including the postcode (this is where the Traffic Case Reference number will be sent)
  • the date and time of the incident
  • the location and postcode of the incident

How long does it take?

It can take up to eight weeks from the date that we bank your payment to process your request.

Costs and payment

To request third party details costs £34.30.

To request a copy of the police report costs £144.90.

To request a Traffic Case Reference number costs £27.10.

You’ll need to include your payment with the completed form.

Please note: We only accept pound sterling and cheques and postal orders must be payable to ‘Mayor’s office for Policing and Crime (MOPAC)’.
